Telehealth Policy

What is telemedicine?
Telemedicine is the delivery of healthcare services, including examination, consultation, diagnosis, and treatment, through electronic communication, information technology, or other means to enable a healthcare provider and you (the patient) at different locations to communicate and share individual patient health information for the purpose of rendering clinical care. Dreem Sleep Clinic of California allows you to receive treatment from a healthcare provider without having to travel to another location or schedule a separate appointment. This helps you avoid long wait times that you might otherwise experience at an in-person visit.
Services Provided
Telemedicine services offered by Dreem Health, and Dreem Health’s engaged providers (our “Providers” or your “Provider”) may include a patient consultation, diagnosis, treatment recommendation, prescription, and/or a referral to in-person care, as determined clinically appropriate (the “Services”).
Sunrise United States, Inc. does not provide the Services; it performs administrative supportive activities for Group and our Providers.
Electronic Transmissions
The types of electronic transmissions that may occur using the telehealth platform include, but are not limited to:

Appointment scheduling;

Completion, exchange, and review of medical intake forms and other clinically relevant information (for example: health records; images; output data from medical devices; sound and video files; diagnostic and/or lab test results) between you and your Provider via:

asynchronous communications;
-
two-way interactive audio in combination with store-and-forward communications; and/or
-
two-way interactive audio and video interaction;
-
Treatment recommendations by your Provider based upon such review and exchange of clinical information;

Delivery of a consultation report with a diagnosis, treatment and/or prescription recommendations, as deemed clinically relevant;

Prescription refill reminders (if applicable); and/or
-
Other electronic transmissions for the purpose of rendering clinical care to you.
If necessary, a healthcare professional may conduct a virtual physical examination of you, including taking video, audio, and/or photo recordings.
Benefits using telemedicine
The benefits of telemedicine include having access to care anywhere you have access to the internet—including from the comfort of your home. Our telemedicine services are available 8 hours a day, 5 days a week.
Telemedicine means you don’t risk exposure to illness in busy waiting rooms, and you do not have to wait several days for an in-person appointment.
Telemedicine also means you do not have to travel great distances to gain access to specialty care that may not be available in your community. It is a convenient access to follow-up care.
Possible risks of using telemedicine
As with any medical treatment, there are potential risks associated with the use of telemedicine. Dreem Sleep Clinic of California believes that the chance of any of these risks occurring is very low.
These risks may include, without limitation, the following:
- Delays in medical evaluation and consultation or treatment may occur due to deficiencies or failures of the equipment or the Internet, which may include poor video and data quality, Internet outages, or other service interruption issues. You may reschedule the visit with your healthcare practitioner should these interruptions occur. In the event of an inability to communicate as a result of a technological or equipment failure, please contact Dreem Health at Phone: (650) 761-4056 and hello@dreemhealth.com.
- Security protocols could fail, causing a breach of privacy of personal medical information.
- Because Dreem Sleep Clinic of California does not have access to your complete medical records, if you do not disclose to your healthcare practitioner a full list of your medical history, including diagnoses, treatments, medications/supplements, and allergies, adverse treatment, drug interactions, or allergic reactions, or other negative outcomes may occur.
- Telemedicine services are NOT emergency services and your Personal Data (as defined in the Dreem Sleep Clinic Privacy Policy) will not be monitored 24/7. If you believe you are experiencing a medical emergency, you should dial 9-1-1 IMMEDIATELY and/or go to the nearest emergency room. PLEASE DO NOT ATTEMPT TO CONTACT US. AFTER RECEIVING EMERGENCY HEALTHCARE TREATMENT, YOU SHOULD VISIT YOUR LOCAL PRIMARY CARE PROVIDER.
⚠️ The care you receive will be at the sole discretion of the healthcare practitioner who is treating you, with no guarantee of diagnosis, treatment, or prescription. The healthcare practitioner will determine whether or not the condition being diagnosed and/or treated is appropriate for a telemedicine encounter via the service.
Our Providers are an addition to, and not a replacement for, your local primary care provider. Responsibility for your overall medical care should remain with your local primary care provider, if you have one, and we strongly encourage you to locate one if you do not.
Security Measures
The electronic communication systems we use will incorporate network and software security protocols to protect the confidentiality of patient identification and imaging data and will include measures to safeguard the data and to ensure its integrity against intentional or unintentional corruption. All the Services delivered to the patient through telemedicine will be delivered over a secure connection that complies with the requirements of the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).
Your rights and acknowledgements
👉 You understand that your healthcare information may be shared with other individuals in accordance with the Dreem Sleep Clinic of California Privacy Policy and regulations or laws in the state or territory in which you are located.
👉 Federal and state law requires health care providers to protect the privacy and the security of health information. I am entitled to all confidentiality protections under applicable federal and state laws. I understand all medical reports resulting from the telemedicine visit are part of my medical record.
👉 Telemedicine may involve electronic communication of your personal medical information to remote healthcare practitioners who may be located outside of your state.
👉 You have the same privacy rights via telemedicine that you would have during an in-person visit.
👉 You understand that no results can be guaranteed or assured—you may not achieve the anticipated benefits of the telemedicine services.
👉 You understand that a variety of alternative methods of medical care may be available to you, and that you may choose one or more of these at any time.
👉 You understand that all information submitted to Dreem Sleep Clinic of California will be part of your medical record, and you have the right to review and receive copies of your medical records. For more information on how to access your medical records, please contact hello@dreemhealth.com.
👉 Persons may be present during the telemedicine visit other than my Provider who will be participating in, observing, or listening to my consultation with my Provider (e.g., in order to operate the telemedicine technologies). If another person is present during the telemedicine visit, I will be informed of the individual’s presence and his/her role.
👉 I understand that I may be asked to provide my identification and confirm my physical location prior to or during the telemedicine visit.
👉 I understand that If I am experiencing a medical emergency, I will be directed to dial 9-1-1 immediately and Dreem Health, including my Provider, is not able to connect me directly to any local emergency services.
👉 This Telemedicine Informed Consent is valid during your entire treatment with Dreem Sleep Clinic of California. If you would like to withdraw consent, you may do so in the course of my care by emailing us at privacy@dreemhealth.com. Your withdrawal of consent will not affect your right to future care or treatment.
No-show cancellation policy
We understand that you may need to cancel an appointment occasionally. If you need to cancel (or reschedule), please do so with at least 48 hours’ notice. You can cancel or reschedule your appointment using the link provided in your confirmation and reminder emails, through the patient app, by emailing us at hello@dreemhealth.com, or by texting at (650) 761-4056. Please note that cancellations by phone are not accepted unless you speak directly with a staff member.
If you do not show up for your appointment, or cancel or reschedule within 48 hours (2 days) of your appointment time, no-show appointments will be subject to a fee of $149 (full consultation cost). No-show fees are the patient’s sole responsibility. If this fee might prevent you from receiving necessary care, please contact us.
We know that unexpected situations sometimes arise. In the case of emergencies or extenuating circumstances, we may waive the no-show fee. Waivers are determined on a case-by-case basis at the practice management's sole discretion.
